RE: Hurley Medical Center Subrecipient Contract

recommendation
BOARD ACTION REQUESTED:
Approval of Subrecipient Contract Amendment between the Genesee County Health Department (GCHD) and Hurley Medical Center. Hurley Medical Center will be paid $91,150.00 per year to provide services for the Healthy Start Initiative. Contract would be valid from May 1, 2024, through March 31, 2029, to be paid from Funding Account- 2211-607.03-801.000, Funding Source Health Resources and Services Administration (HRSA).

Case management and care coordination services are provided by a multidisciplinary team of certified nurses, social workers, and dieticians. Services provided by the team function to decrease maternal and infant mortality rates in Genesee County.
DISCUSSION:

During home visits, participant are provided with case management services, health education, and referrals for current needs. On average participants will receive one to two home visits per month but can receive up to four depending on their risk assessment. Genesee County Healthy Start received continued federal funding for funding cycle May 1, 2024, through March 31, 2029. No county appropriation is needed.
