RE: Subrecipient Contract for Reverence Home Health and Hospice DBA Ascension at Home

recommendation
BOARD ACTION REQUESTED:
Approval of Subrecipient Contract Amendment between the Genesee County Health Department (GCHD) and Reverence Home Health and Hospice DBA Ascension at Home. Subrecipient will be paid $150,000.00 per year to provide services for the Healthy Start Initiative. Contract would be valid from May 1, 2024, through March 31, 2029 to be paid from Funding Account- 2211-607.01-801.001, Funding Source Health Resources and Services Administration (HRSA).
BACKGROUND:
HRSA requires all Healthy Start grantees to provide case management/care coordination services to all program participants. Due to staffing capacity and specialization of maternal child health home visiting services, two local hospitals have provided case management/care coordination services for the Genesee County Healthy Start Initiative since 2001. Case management and care coordination services are provided by a multidisciplinary team of certified nurses, social workers, and dieticians. Services provided by the team function to decrease maternal and infant mortality rates in Genesee County.
DISCUSSION:
Health Start is an infant mortality reduction program that uses a multidisciplinary approach to provide home visiting services to families in the Genesee County community. During home visits, participants are provided with case management services, health education, and referrals for current needs. On average participants will receive one to two home visits per month but can receive up to four depending on their risk assessment. Genesee County Healthy Start received continued federal funding for funding cycle May 1, 2024, through March 31, 2029. No county appropriation is needed.
