RE: Residential Substance Abuse Treatment (RSAT) Matching Grant

recommendation

BOARD ACTION REQUESTED:
Approval of a request to designate $114,000 (25% Matching Grant) from the opioid settlements’ Core Remediation for the remainder of Fiscal Year 2024, ending September 30, 2024.

This RSAT funding opportunity will allow residents in the Genesee County Jail to begin receiving Medicated Assisted Treatment (MAT)/Medicated Opioid Use Disorder (MOUD) while in the GCJ, and seamlessly be transitioned upon release to a substance use disorder treatment provider in the community.

BACKGROUND:
Currently, the contracted medical provider in the GCJ, Advanced Correctional Healthcare (ACH) provides Naltrexone (Vivitrol) on a limited basis. While this benefits a limited number of people, the Genesee County Sheriff’s Office (GCSO) desires to increase access to Naltrexone and Buprenorphine products (Suboxone, Sublocade, and/or Brixadi) in the GCJ.

In partnership with the Greater Flint Health Coalition (GFHC), New Paths, Inc. (NPI), and Region 10 Pre-Paid Inpatient Health Plan (PIHP), and Health Management Associates, the GCSO has developed a robust vision for piloting a program to begin offering these medications for individuals entering the GCJ that were receiving MAT/MOUD prior to incarceration.

These plans include the integration of assessment tools upon admission to identify individuals in need of continuing MAT/MOUD, processes for prescribing the necessary medications, ensuring each client is enrolled in jail-based outpatient treatment and receiving Peer Recovery Coaching throughout the service delivery period, and developing individualized aftercare plans with each participant prior to reentry (RSAT Scope and Budget Proposal Attached).

DISCUSSION:
Genesee County is now receiving funding from Opioid Settlements. The OEC has been meeting regularly to determine best practices for spending these dollars.  To allow GFHC to accept the RSAT funding being issued by the State of Michigan, a 25% matching grant is required.  The RSAT scope of work is an identified core remediation service listed within Exhibit E (Attached).

The goal for the 6-month pilot phase of this project will be to establish systems that enable the GCSO in partnership with the GFHC, NPI, and ACH, to identify people in need of MAT/MOUD continuations upon admission to the GCJ, establish processes for facilitating service delivery, and ultimately serving approximately 50 individuals in the first 6-months of expanding MAT/MOUD services. Health Management Associates will provide technical assistance and training as needed to the jail-based MAT/MOUD implementation team throughout this 6-month pilot phase.

For the remainder of fiscal year 2024, it is projected $555,463.00 will be received until September 30, 2024.  To receive this funding opportunity, $114,000.00 is being requested for Core Remediation from the Opioid Settlements.

Grant Budget will be as follows:

Match request to Genesee County:  $114,000
Other match funds provided via GFHC and Health Management Associates: $25,000
25% Match Total:  $139,000
RSAT Funds Total:  $416,463
Total Project Budget with RSAT and Match:  $555,463.00

Funds will be leveraged to build staffing capacity for the GCSO, cover subcontractor NPI costs for baseline lab work supplies and costs of buprenorphine and naltrexone medications prescribed for verified MAT/MOUD candidates lodged in the GCJ, and support project management/administration provided by GFHC.

There would be no cost to the general fund.

CONFORMITY TO COUNTY PRIORITIES:
The Genesee County Board of County Commissioners have committed to creating a Healthy, Livable & Safe Community.
